Our boutique style practice allows for ample doctor-patient interaction time throughout the entire visit so that each patient receives the most comprehensive care possible. We offer a personalized approach to healthcare where patients are given our full attention in a comfortable, modern office environment.

INSURANCE COVERAGE &

PAYMENT OPTIONS

Affordable Healthcare

We are a participating provider with Medicare and can accept most non-HMO insurance plans. All insurance coverage will be verified prior to your first visit to determine what benefits are available to you. Personal injury claims are accepted for those who have been involved in a car accident, all Florida auto insurance policies carry a minimum of $10,000 of personal injury protection for treatment expenses. Insurance verification requests may be sent via email to ElanVitalHealth@aol.com. Please include the name of the insurance carrier, the ID # printed on the card, your full name and your date of birth when requesting insurance verification.

No insurance? No problem!

Our office is dedicated to helping you receive the care you need at a cost you can afford. For those with no insurance or restricted insurance coverage (high deductibles or limited benefits) our office participates with the ChiroHealthUSA discount network to offer affordable self-pay rates. New patient appointments are often available on the same day, or will be scheduled as soon as possible (typically within 1-2 days) to avoid any prolonged suffering. Please let us know if you've been involved in a car accident or experienced an injury when scheduling an appointment!

Our office works on appointments only to ensure each patient is allotted the proper amount of time, make sure to schedule an appointment in advance as walk-ins cannot be accommodated. Our new, modern office is conveniently located inside the Regus Executive Suites at the Galleria Mall in Fort Lauderdale.
